Description
The Division of Geriatrics seeks a Nurse Practitioner to support the UCSF 360 Wellness Center consult service.

UCSF 360 Wellness Center was created in 1984 to offer a range of high-quality services in a single location for people living with HIV/AIDS, and this position requires optimizing the care of older adults in the 360 Clinic when consults are requested by providers.

The clinical work involves providing consultation services from Geriatrics to the 360 Clinic by completing structured assessments, patient exam, and making recommendations to the clinic and Primary Care Physicians (PCPs) for ongoing care/ home management as indicated.

It involves coordination with pharmacists, 360 clinic providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals and/or services to create an optimal discharge or ongoing plan of care for the patient.

The position does not involve direct clinical management of patients in the 360 Clinic.

The committed hours are based on ad hoc needs of the 360 Clinic through scheduled consults, which may be conducted in person or via telemedicine.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.
