About the Department
The Community Health Worker (CHW) will be a member of the Comprehensive Care Program's (CCP) clinical team, specifically focused in providing direct care to patients enrolled in the Comprehensive Care, Community and Culture Program (C4P).
The CHW will work across neighborhoods in the Chicago's South Side and surrounding neighborhoods to address social determinants of health and assist CCP patient's in the following area's: adherence support, assistance with healthcare navigation, patient education and social service delivery.
Responsibilities:
- Proactive in learning about services, conducts regular canvassing of resources available in the surrounding communities, builds relationships and serves as a liaison between patients and community agencies.
- Completes Social Determinants of Health (SDOH) assessments and provides support and interventions for a panel of chronically ill patients with chronic social needs.
- Meets directly with patients and their family members in their home, in the community or in the clinic setting to assess and address unmet social needs such as but not limited to: transportation, healthcare navigation, housing, food insecurity, social engagement, public benefits.
- Documents patient encounters in electronic medical record and other data entry systems in a timely manner.
- Acts as a collaborative partner with members of the healthcare team to facilitate appropriate transitional care plans and care coordination efforts.
- Understands and abides by Mandated Reporting Policies and facilitates the involvement of clinical team members to make appropriate referrals to Illinois Department of Children and Family Services or Illinois Department of Aging when there is concern for suspected abuse or neglect.
- Participates in program process improvement, community wellness events and research activities.
- Assists with public presentations and community outreach events.
- Assists with data entry and reports on results.
- Assists in analyzing data. Assists with preparation of reports, manuscripts and other documents.
- Performs other related work as needed.
Education:
Minimum requirements include vocational training, apprenticeships or the equivalent experience in related field (not typically required to have a four-year degree).

- Experience working in medical settings on an interprofessional team as a community health worker, health navigator, or comparable role.-
- Experience and knowledge of the city of Chicago/South side social service resource landscape.
