- Evaluate and assess patients to determine physical therapy needs and develop treatment plans
- Provide one-on-one therapy sessions in the comfort of patients' homes
- Work with patients to set achievable goals and track progress
- Collaborate with other healthcare professionals to provide comprehensive care
- Use innovative and effective techniques to help patients achieve their maximum physical potential
- Master's or Doctoral degree in Physical Therapy
- Active state license as a Physical Therapist
- At least 1 year of experience in a clinical setting preferably home health
- Strong communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
