General Description


Under the direction of the Director of Nursing, assist in directing and coordinating the daily operations for the assigned hall residents in accordance with state and federal rules, regulations, and standard clinical practices.


Hours of Work

  • Hours are generally 8hour days Monday through Friday between the hours of 7:00 a.m. and 5:30 p.m. Variations of these hours may occur as the DON would foresee it to be necessary either to meet the needs of the staff or residents.
  • The Resident Care Coordinator will be responsible to be on call on the designated rotation and may be called in the event of an emergency on any other off hours.

Job Responsibilities, Duties and Authority:


  • Develop and maintain a good working rapport with interdepartment personnel, as well as other departments within the facility.
  • Report all unsafe/hazardous conditions, defective equipment, etc., to your supervisor immediately. Submit work orders as needed.
  • Maintain confidentiality of all pertinent resident care information to assure resident rights are protected.
  • Assist in assuring the provision of treatments, medications, and nursing services according to a resident's care plan and physician directed orders.
  • Update Care plan for daytoday changes; Review staff documentation to ensure continuity of care and appropriate utilization of the Care Plan. Write Care Plan notes, Care plan section, and nurse's notes.
  • Utilize interdisciplinary input together with resident and family members' input to produce an individualized Plan of Care for each resident.
  • Maintain appropriate signed consents.
  • Address any family concerns in conjunction with social services.
  • Address and report any care concerns and work to resolve those concerns.
  • Be mindful of regulations related to antipsychotics, required dose reduction attempts and documentation of such.
  • Ensure behavioral monitoring is in place for residents on psychotropic medications.
  • Ensure residents have appropriate diagnoses for catheters, psychotropics, etc.
  • Monitor resident care indicators including, but not limited to, pressure areas, anticoagulation, falls, chemical and physical restraints, and infections together with assigned nursing staffassist with 802 completion/updates as needed.
  • Collaborate with interdisciplinary team regarding therapeutic interventions.
  • Ensure lab orders are followed.
  • Complete all appropriate assessments & observations.
  • Assist with training for those working on the unit.
  • Participate in department meetings and maintain inservice requirements.
  • Assist with resident care in emergency situations.
  • Assist with resident admission process and complete admission audits.
  • Assist with coordination of resident discharge planning and discharge education in collaboration with social services.
  • Will be visible and available on the unit and will coach/mentor staff as needed.
  • Utilize computer system to produce all required documentation and reports.
  • Follow up with physician and family members as needed to achieve treatment goals.
  • Conduct and Record Resident Covid 19 testing according to outbreak status.
  • Complies with HIPAA regulations.
  • Duties as assigned.

Working Conditions:


  • Must be able to spend approximately 70% of the time walking, standing, pushing, pulling, bending, reaching, or lifting; Must be able to spend approximately 30% of the time sitting.
  • Is involved with residents, family members, personnel, visitors, government agencies and personnel, etc., under all conditions and circumstances.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to use a computer, phone etc. for extended periods of time.
  • Exposure to blood or to the body fluids, fumes, or airborne particles and toxic or caustic chemicals.

Minimum Training, Experience and Certification Requirements:

  • A graduate of an accredited college with focus in social services or nursing.
  • Preferably 5+ years' experience with the geriatric population with sensitivity to the total needs of the older adult.
  • Preferably 2+ years' experience working with people with Alzheimer's disease or related illness.
