- Coordinates continuity of patient care with patients and families following hospital admission and discharge, and for clinics ER visits, and PCP visits. Facilitates hand-off throughout the continuum of care.
- Manages high-risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting.
- Provides and facilitates education of parents/families of patients following admission and coordinates discharge planning.
- Serves as resource for team promoting advocacy for patients, families, and Arkansas Children's Team.
- Conducts comprehensive, preventative screenings for patients and/or assists all support staff in daily patient interactions as needed.
- Consistently provides safe and effective nursing care at a proficient level.
- Proficiently assesses increasingly complex patients and serves as a resource to others to ensure developmentally appropriate and culturally sensitive care.
- Provides individualized patient and family education at a proficient level in increasingly complex care situations, and serves as a resource in guiding others.
- Proficiently mentors, coaches, precepts, and teaches healthcare professionals and students in the care of increasingly complex patients.
