The role of the Occupational Therapist within the IDT is pivotal, involved in care planning and close collaboration with the clinical team.

The Occupational Therapist plays a crucial role in providing restorative and rehabilitative occupational therapy services both at the Center and in a home setting. Working alongside the Primary Care Physician, rehabilitation team, and other IDT members, the goal is to enhance participant independence, ensure safety, and facilitate functional improvement.

Key Responsibilities:

Conduct assessments to identify participant needs, preferences, and goals, actively engaging in IDT meetings to formulate care plans.
Deliver and document interventions as per care plans, promoting muscle re-education, cognitive function, balance, and more to enhance independence in daily activities.
Supervise Therapy Assistants, offering necessary training for implementing Functional Maintenance Programs (FMP).
Provide guidance and training to staff, participants, and caregivers in Occupational Therapy practices.
Collaborate with IDT members to ensure seamless care transitions between different settings and participate in end-of-life care coordination.

Requirements:

Bachelor's Degree in Occupational Therapy from an Accredited Program following AOTA & ACOTE standards.
Minimum of two (2) years of occupational therapy experience with frail or elderly populations in a clinical setting.
At least two (2) years of supervisory experience, showcasing mentoring and team development skills.
Registration from the National Board for Certification in Occupational Therapy.
Valid state licensure or eligibility for licensure as per state regulations.

The base salary range for this role is $99,309.58—$119,717.49 per year, supplemented by bonuses and equity. WelbeHealth offers a competitive rewards package encompassing 401k matching, healthcare benefits, and more, with actual compensation varying based on experience and qualifications.
