Within an Integrated Behavioral Health Model of Care, the Licensed Social Worker (LMSW or LCSW) will provide clinical treatment to patients and families who are experiencing various mental health disorders including depression and anxiety.

The Social Worker's major goal is to help patients modify the impact of these problems and assist patients with identifying effective coping mechanisms and lifestyle changes.

Responsibilities and Requirements:

Utilize Evidenced-Based Assessment tools and Treatment methods that are based on research and evidence gathered in clinical assessments, evaluations of outcomes and practice
Collaborate with an Interdisciplinary Team comprised of Primary Care Doctors, Psychiatrists, Nurses, and other internal and external professionals to improve the quality of patients' lives
As needed, the Social Worker will also provide patients with assistance with Social Determinants of Health (concrete services) and make appropriate referrals to ensure patients' safety at home and in the community

Monday - Friday 8:30a - 5:00p
