Position Summary:


As an integral member of our team, the Certified Home Health Aide (CHHA) works under the supervision of a registered nurse and provides personal care services to our patients, as well as assist with patient's activities of daily living, in accordance with the patient's plan of care.


FUNCTIONS & RESPONSIBILITIES

  • Reports changes in the patient's conditions and needs to the RN Case Manager or Director
of Patient Care Services.


  • Observes and reports signs and symptoms of discomfort including presence of pain.
  • Provides or assists patient in bathing, care of mouth, skin, and hair.
  • Assists patients in and out of bed and assisting with ambulation.
  • Turns patients in bed and positions for comfort.
  • Assists with prescribed exercises which patients and home health aides have been taught by
appropriate health personnel.


  • Assists patients with feeding.
  • Assists patients to bathroom or in using bedpan or urinal.
  • Changes bed linen.
  • Observes for safety hazards in the patient's and organization's environments and takes
appropriate steps to reduce or eliminate them.


  • Attends and participates in in-services, staff meetings.
  • Completes all documentation in a timely manner.
  • Complies with policies and procedures, laws and regulations and standards of practice.
  • Performs other duties as assigned by the RN Case Manager or Director
of Patient Care Services

Duties:

  • Provide personal care assistance to patients in their homes
  • Assist with activities of daily living, including bathing, dressing, and grooming
  • Monitor and record vital signs, such as blood pressure and temperature
  • Administer medications as directed by a nurse or healthcare professional
  • Assist with mobility and transfers, using proper lifting techniques
  • Provide companionship and emotional support to patients
  • Help with meal preparation and feeding
  • Perform light housekeeping tasks, such as cleaning and laundry
  • Assist with hospice care and endoflife support
  • Provide care for patients in assisted living facilities

Skills:


  • Strong ability to lift and transfer patients safely
  • Proficient in taking vital signs accurately
  • Knowledge of patient care techniques and protocols
  • Experience in meal preparation and feeding assistance
  • Understanding of hospice care principles and practices
  • Familiarity with medical terminology and documentation procedures
  • Ability to provide care for patients with dementia or cognitive impairments

Job Types:
