Position Summary:
As an integral member of our team, the Certified Home Health Aide (CHHA) works under the supervision of a registered nurse and provides personal care services to our patients, as well as assist with patient's activities of daily living, in accordance with the patient's plan of care.
FUNCTIONS & RESPONSIBILITIES
- Reports changes in the patient's conditions and needs to the RN Case Manager or Director
- Observes and reports signs and symptoms of discomfort including presence of pain.
- Provides or assists patient in bathing, care of mouth, skin, and hair.
- Assists patients in and out of bed and assisting with ambulation.
- Turns patients in bed and positions for comfort.
- Assists with prescribed exercises which patients and home health aides have been taught by
- Assists patients with feeding.
- Assists patients to bathroom or in using bedpan or urinal.
- Changes bed linen.
- Observes for safety hazards in the patient's and organization's environments and takes
- Attends and participates in in-services, staff meetings.
- Completes all documentation in a timely manner.
- Complies with policies and procedures, laws and regulations and standards of practice.
- Performs other duties as assigned by the RN Case Manager or Director
Duties:
- Provide personal care assistance to patients in their homes
- Assist with activities of daily living, including bathing, dressing, and grooming
- Monitor and record vital signs, such as blood pressure and temperature
- Administer medications as directed by a nurse or healthcare professional
- Assist with mobility and transfers, using proper lifting techniques
- Provide companionship and emotional support to patients
- Help with meal preparation and feeding
- Perform light housekeeping tasks, such as cleaning and laundry
- Assist with hospice care and endoflife support
- Provide care for patients in assisted living facilities
Skills:
- Strong ability to lift and transfer patients safely
- Proficient in taking vital signs accurately
- Knowledge of patient care techniques and protocols
- Experience in meal preparation and feeding assistance
- Understanding of hospice care principles and practices
- Familiarity with medical terminology and documentation procedures
- Ability to provide care for patients with dementia or cognitive impairments
