- Qualified candidates must have a Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy

- Candidates should also be self-motivated, have good communication, organizational, and interpersonal skills, and possess or be eligible for licensure in New Jersey State.

- Providing pharmaceutical care to patients with a diagnosis of congestive heart failure by assessing pharmacotherapy needs for both the inpatient and outpatient settings.
- Performing medication reconciliation, and providing education to patients transitioning to home.
- Make recommendations to providers that will facilitate optimizing the patient's medication therapy.
- Evaluate patients' medication adherence and compliance post-discharge by evaluating patient's access to and understanding of their medications for congestive heart failure.
- Serve as a pharmacotherapy resource for licensed independent practitioners, nurses, and other healthcare disciplines.
- Work collaboratively and communicate timely and effectively with the Transitions of Care nurse practitioner and other members of the health care team.
- Participate in performance improvement and quality initiatives designed to improve clinical outcomes in congestive heart failure patients.
- Support the PGY1 Pharmacy Residency Program and other department/hospital initiatives as needed.
