About Optima
Care Manager Responsibilities:
- Manage a roster of patients who are enrolled in the Hybrid Patient Monitoring program (RPM).
- Act as a direct liaison between the patient and the office/provider.
- Scheduling and confirming appointments in office for the patient.
- Have a full understanding of each patient's individual care plan and making sure they are following it.
- Record patient data daily and alert healthcare providers of abnormal reports.
Care Manager Qualifications:
- At least 1 year of clinical work experience as a Medical Assistant
- At least 1 year of EMR experience; eClinicalWorks preferred
- High level of professionalism
- Must live in Arizona
- Excellent written and verbal communication skills
- Strong customer service skills
- Medical Assistant Course Completion Diploma _or_ CMA designation required
