Description
EWPS is seeking a highly qualified Transition Coordinator for its 18-22 year-old program located at East Windsor High School. The program offers a fully equipped kitchen with washer and dryer for authentic life skills instruction. The ideal candidate will work effectively with students with disabilities, setting challenging and realistic goals for them. Team work and the ability to supervise staff effectively is essential.


Responsibilities include:

Supervise students on already established job sites in the communityCreate and implement lesson plans for vocational and life skillsSupervise Job Coaches and ParaprofessionalsCoordinating transportation to/from work sites and to/from schoolSecures new school and community work-based learning opportunities to match students interests and abilities.

Communicates with parents/families regularly to support students' success and their transition to state agency adult services upon aging out of special educationNetworks with all appropriate community agencies and organizations.

Reports out at PPT meetings on student IEP goals/objectivesSchedules PPT meetingsQualifications:
CT Special Education Teacher certification (preferred but not required)

Must be able to work with colleagues effectivelyExcellent attendanceReport to:
