- Provides overall personal care to patients following the written plan of care. Allows for patient input and preference in this plan.
- Assists patients with or provides patients with personal care as directed. Ensuring safety of patient.
- Assists patients in ambulating as directed by nurse. Performs simple procedures as an extension of therapeutic services including P.R.O.M. and A.R.O.M.
- Transfers patients from bed to chair/wheelchair and back using proper lifting and transferring techniques and/or equipment. (Gait belt, Hoyer lift, bench, walker, cane, wheelchair.)
- Provides simple meal preparation, linen change, personal laundry, tidying bedroom, tidying bathroom, dishwashing, and light housekeeping as directed.
- Current Certified Home Health Aide - Iowa Registry required.
- Expected to take weekend calls, holiday calls, and make home visits as required.
- Must apply and obtain Illinois licensure.
- Must provide current, valid driver's license.
- Must provide current, valid auto insurance.
- One-year health aide experience required; Home Care/Hospice experience preferred.
- High school graduate or equivalent.
- Must be BLS certified within 3 months of hire.
- Must possess good oral, written, communication, and observational skills.
- Must be able to complete work efficiently, effectively and independently.
- Hospice training if applicable.
- Must be comfortable operating in a collaborative, shared leadership environment.
- Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
