- Effectively and efficiently prioritizes visit content and plans visits based on client capabilities, condition, preferences (when possible), physician orders and available resources.
- Accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Assesses the client 's condition and needs and determines the agency's ability to meet those needs in the home care/hospice setting.
- With the physician, office care manager and client, develops appropriate assessment-based plan of treatment including: attainable episode goals and agency services needed to improve and/or stabilize client's medical condition and promote independence. Participates in timely revision of the plan of treatment as appropriate.
- Makes appropriate referrals for evaluation/care to other disciplines and services, and assists in coordinating care with others to ensure effective and efficient care is provided.
- Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in formulating nursing interventions and making recommendations to the physician and client.
- Reports changes in client condition as appropriate and in a timely manner, to the client's physician and/or Case Manager/designee, and obtains orders for changes in the plan of treatment to respond to the client's condition.
- Initiates and assures implementation of procedures, orders for medication and treatments as prescribed by the physician in accordance with the plan of treatment.
- Accurately documents observations, interventions and evaluations pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system
- Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
- Active registration for Registered Nurse - Board of Nursing, State of Iowa or Illinois
- An Illinois or Iowa (whichever is not currently held) Registered Nurse - Board of Nursing, license must be applied for within 30 days of hire and secured within 6 months of hire.
- Associate's degree or diploma from an accredited school of nursing.
- Baccalaureate degree in nursing preferred; must obtain within 6 years of hire, effective 04/01/2012.
- Minimum of one (1) year experience as a professional acute care nurse preferred.
- Home healthcare experience preferred.
- Valid driver's license, current auto insurance and reliable transportation to and from work site are required.
- Must be comfortable operating in a collaborative, shared leadership environment.
- Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
