- Conduct Adult/Geriatric Assessments, medical history review, diagnosis and treatment, health education, and clear documentation as per Matrix guidelines
- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings like Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree OR equivalent experience with NP licensure
- Current RN and NP licensure with prescription authority or eligibility for prescriptive authority
- Board certified in a Matrix approved specialty
- BLS, ACLS or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with health record software
- Excellent communication skills
- Comfortable with change and flexibility
- Valid driver's license, car access, and adequate insurance
- Travel as needed to meet patients at their location

Nurse Practitioner – PRN/Part Time

About the Position:
Matrix providers conduct comprehensive health assessments during visits, gaining valuable insights into members' overall health beyond routine office visits. This approach helps in enhancing the quality of care and addressing multiple care gaps in a single visit.

Matrix Provider – What to Expect:
Nurse Practitioner - PRN/Part-Time - Home Risk Assessments
Type: PRN/Part-Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (county-based) per completed Assessment
Location: Home, SNF, Telehealth, and other community environments in Dayton, Ohio
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: 401(k) and limited Voluntary Benefits for PRN/Part-Time providers
Mileage Reimbursement plan
