    Nurse Practitioner – PRN/Part Time

    Home Risk Assessments

    About the Position:

    Matrix providers conduct comprehensive health assessments during visits, providing valuable insight into members' health, medication adherence, and social environment. This approach improves care quality and closes multiple care gaps in a single visit.

    Responsibilities

    Matrix Provider – What to Expect:

    • Conduct assessments, medical history, diagnosis, treatment, and clear documentation
    • Work collaboratively with healthcare professionals and caregivers
    • Provide services in various settings including home visits, skilled facility visits, and telehealth visits

    Nurse Practitioner - PRN/Part Time - Home Risk Assessments

    Type: PRN/Part Time - Flexible-Nurse Practitioner

    PAY: $90.00 or $95.00 per completed Assessment

    Location: Home, SNF, Telehealth in Dayton, Ohio

    Hours: Days, Evening, Nights, weekend, Flexible Scheduling

    Benefits: PRN/Part Time providers have 401(k) and voluntary benefits

    Mileage Reimbursement plan

    Qualifications

    Must Haves:

    • Master's Degree or equivalent experience
    • Current RN and NP licensure, board certification, and relevant certifications
    • Minimum 1 year experience as a Nurse Practitioner
    • Strong communication and computer skills
    • Comfortable with frequent changes

    Travel Requirements:

    • Must have valid driver's license and adequate automobile insurance
    • Travel is required to meet patients' needs
