We are seeking a skilled and compassionate Physical Therapist to join our team at WayPoint Home Health Care. The ideal candidate will play a crucial role in providing quality physical therapy services to our patients, promoting recovery, mobility, and overall wellness.

Responsibilities:

Conduct thorough assessments of patients' physical conditions and develop individualized treatment plans
Provide hands-on therapy, exercises, and interventions to improve patients' strength, mobility, and function
Educate patients and their families on home exercise programs and proper body mechanics
Monitor and document patients' progress, adjusting treatment plans as necessary
Collaborate with a multidisciplinary team to ensure coordinated and comprehensive care for patients

Qualifications:

Valid state license to practice as a Physical Therapist
Strong clinical skills and knowledge of various physical therapy techniques
Excellent communication and interpersonal skills
Ability to work effectively both independently and as part of a team
