Job Summary:
Duties:
- HMIS entry of client
- Conduct assessments to identify clients' needs and develop individualized service plans/ goals
- Provide crisis intervention and support to clients facing challenging situations
- Coordinate and monitor services from various agencies to ensure comprehensive care for clients
- Supervise and monitor client progress towards established goals
- Collaborate with healthcare providers, social workers, and other professionals to advocate for clients' needs
- Assist clients in accessing services such as behavioral health, addiction counseling, and workforce development programs
- Manage cases involving, Section 8 housing, Medicare, and other relevant programs
Requirements:
- AA degree or bachelor in social work, psychology, or related field; preffered but not required.
- Proven experience in case management or related field
- Strong knowledge of crisis management techniques and behavioral health interventions
- Familiarity with child welfare services and workforce development programs
- Excellent communication and interpersonal skills
- Ability to work effectively in a fastpaced environment with diverse populations
