Job Summary:

Duties:


  • HMIS entry of client
  • Conduct assessments to identify clients' needs and develop individualized service plans/ goals
  • Provide crisis intervention and support to clients facing challenging situations
  • Coordinate and monitor services from various agencies to ensure comprehensive care for clients
  • Supervise and monitor client progress towards established goals
  • Collaborate with healthcare providers, social workers, and other professionals to advocate for clients' needs
  • Assist clients in accessing services such as behavioral health, addiction counseling, and workforce development programs
  • Manage cases involving, Section 8 housing, Medicare, and other relevant programs

Requirements:


  • AA degree or bachelor in social work, psychology, or related field; preffered but not required.
  • Proven experience in case management or related field
  • Strong knowledge of crisis management techniques and behavioral health interventions
  • Familiarity with child welfare services and workforce development programs
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fastpaced environment with diverse populations

Pay:
$ $15.00 per hour
