Who We Need

Immediately Hiring a Program Coordinator
Maintain and ensure the maintenance of quality programs which promote person-centered planning and developmentally/age-appropriate practices.

Provide assistance in acting as a liaison for individuals and family concerns and serve as an advocate for individuals, family members, and funding sources.

Responsible for the overall performance in providing operational, administrative, and leadership and coordination of services to members.

MINIMUM QUALIFICATIONS:

Requires any combination of training, education, and experience listed below:

  • Requires a bachelor's degree in human services or related field plus two (2) years' experience working with individuals with disabilities; equivalency of education and experience is acceptable.
  • One (1) year of supervisory experience in a behavioral health/healthcare setting required.
  • Satisfactory work records, as determined by the Human Resources Department.
  • Successful completion of Copa's training program, meeting the requirements of contracted funding sources.
  • Ability to meet Copa's driver authorized driving requirements, set forth by Copa Health's insurance company. Including AZ Driver's license upon hire, 39month MVR, proof of insurance and vehicle registration, if applicable.
  • Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks, including drug testing.
  • Preemployment Physical Clearance required

JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)

  • Monitor the provision of quality services within the CDS Department.
  • Coordinate information and contribute to the successful resolution of challenges interdepartmentally and intradepartmentally.
  • Document/record/file and maintain departmental/member information electronically or written as needed.
  • Monitor and ensure that services provided for members are developmentally/age appropriate, culturally sensitive and show respect and dignity. In addition, services support members' rights, personcentered plan, choice, independence, new skill development, social development, and volunteer, employment, and community engagement opportunities.
  • Participate with tours, intakes, discharges, and personcentered services plan meetings. Present necessary information at staffing's, and act as a liaison for family concerns, as necessary and complete all required documents to be processed.
  • Develop staff assignments to meet members ratio requirements, communicate with Transportation and Coverage Coordinator regarding staffing assignments for ratios and coverage need.
