Family Nurse Practitioner, board certified. Specialization in Geriatrics is preferred.
Licensure:
Illinois; additional state licenses preferred
Experience;
3 or more years of experience working in a nursing home. Experience working in Integrated Care is a plus. If not, has to be willing to learn about chronic care management and integrated care models.
Job Duties:
- Willingness to work in a nursing home environment
- Conduct intake assessments and come up with a care plan
- Be a liason between the primary care physicians, their nurse practitioners, nursing home staff, patients and their families
- Be open to providing care management services for patients with chronic medical and mental health diagnosis
- Create a problem list, measurable goals for chronic medical diagnosis
- Willingness to work with patients and their families to provide education based on their diagnosis
- Williness to work with primary care physicians and nurse practitioners
- Williness to learn about lifestyle and medicine coaching
- Willingness to supervise and sign off style medicine coaches (Bachelor or Masters trained employees) that provide various CCM services that focus on topics like sleep, nutrition, physical activity, mindfullness, substance use (alcohol, tobacco), and social determinants of health
- Collaborates with various health care providers treating the patients
- Documents in the electronic health record
Knowledge, Skills and Abilities:
- Has a basic understanding of the physiology and pathology of chronic medical diagnoses, and their treatments (for example obesity, hypertension, diabetis, asthma, etc.)
- Knowledge of lifestyle medicine and health coaching is preferred but not required.
- Excellent verbal and written communication skills required
- Willingness to learn our home grown EHR system
Job Type:
Full-time, Monday thru Friday, 40 hours a week, all on site work, no weekends, no on call.
Salary
$120,000 base salary with benefits
Pay:
$100, $120,000.00 per year
