Position Purpose:
The Community Support Services Manager will be responsible for the daily operation of case management activities, implementation of new programs and compliance with policies and procedures.
The position will be accountable for establishing or refining productivity standards for the team and ensure compliance with department policies and procedures in collaboration with the department Director.
The manager will serve as a resource to Families Together of Orange County community health center's staff, providers and community partners.
The Community Support Services Manager will promote collaboration, be up to date on best practices and establish and strengthen community partnerships.
Core Duties and responsibilities, include but are not limited to:
- Support the vision, mission and goals of Families Together of Orange
- Assist the team and department in carrying out the department and program responsibilities and collaborate with others to support short
- Mentors, trains and manages a team of nonclinical case managers
- Oversees the process, management and tracking of case management referrals
- Manages the data tracking and reporting and maintains accurate program documentation
- Assists with the development and oversight of case management programs, trainings, work plans and reports
- Collaborate with director, team and other organization colleagues to develop and improve processes and workflow
- Assists in developing and revising policies and procedures to ensure compliance with program guidelines
- Assist with assessment and other process improvement strategies to enhance the effectiveness of the case management department and or programs
- Provide direct case management and handle complex case referrals
- Maintains knowledge of current and best practices and industry trends
- Establishes and strengthens relationships with community and housing service providers as well as with private property manager/owners
- Participate in internal and external committees, coalitions and events such as community and health resource fairs, as appropriate
- Complete other projects and duties as assigned

Education and Training:
- Bachelor's Degree in Human Services or Social Work, Master's preferred
- Bilingual: Spanish (required)
Qualifications, Skills, and Experience:
- Five (5) years' experience in case management
- Previous staff management or supervisory experience, at minimum three (3) years
- Experience working with diverse communities and unhoused or atrisk populations
- In depth knowledge and experience of Orange County community, homeless and social service providers and resources
- Experience or familiar with County databases (i.e. Homeless Information Management System, Coordinated Entry System)
- Highly skilled interpersonal, with excellent relationship skills including conflict resolution with colleagues
- Excellent communication skills, both written and verbal
- Proficient in MS Office and Outlook
- Valid CA Driver License
- Utilize Trauma Informed Care Approach
