Job Summary and Qualifications:

***: The licensed clinical social worker is responsible for the provision of psychosocial assessment for pre and post-operative patients undergoing bariatric surgery. The social worker will be act as a multidisciplinary team member collaborating with surgeons, nurse practitioners, dietitians, nurses etc. They will assist in evaluating readiness for surgery and ability to comply with pre and post-operative surgical requirements.


SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:


  • Demonstrates good skills in all forms of communication. Works well with others in a spirit of teamwork and cooperation.
  • Collaborates with providers to develop and implement treatment and intervention programs that patient can understand and comply with
  • Maintains education and development appropriate for position.
  • Adheres to established hospital and department guidelines and protocols. Supports hospital mission, vision, and values.
  • Supports the NH Market by providing psychosocial and initial assessments for all bariatric patients
  • Provides postoperative bariatric assessments and care plans as appropriate.
  • Coordinates treatment planning and maintains ongoing contact with outpatient providers for continuing care of patient.
  • Identifies cultural and religious needs, language barriers, etc., and modifies plan of care as necessary to accommodate these needs.
  • Provides direct counseling with individuals and families
  • Serves as an advocate for patients, family and significant other in regards to decisions regarding plan of care
  • Serves as professional and clinician role model to staff in interactions with patients, families, and staff.
  • Provides education to the patient and/or when appropriate, his/her significant other(s) that can enhance their knowledge, skills, and those behaviors necessary to fully benefit from the healthcare interventions provided by the organization.
  • Attends monthly bariatric committee meetings
  • Participates in monthly pre and post operative support groups

What qualifications you will need:


  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
  • Masters Degree
  • Clinical Social Worker (MSW), or Licensed Clinical Social Worker (LCSW)
