  • Location: [Skokie, IL
  • Full Time; 40 hours per week
  • Hours: Monday-Friday, 8:00 am4:30 pm, rotating every 3rd weekend, 23 holidays per year
  • Required Travel: NA

What you will need:

  • Current RN license, BSN
  • Minimum of 2 to 3 years nursing experience in a hospital setting or office setting.
  • Experience with Discharge Planning with outpatient care coordination Preferred
  • Orthopedic Experience Preferred
  • Excellent interpersonal and customer service skills.
  • Strong problemsolving skills with a flexible and creative approach.
  • Ability to work independently
  • Excellent oral and written communication skills

What you will do:

  • Responsibility for a group of orthopedic patients during a defined episode of care.
  • Connects with patient at the time of diagnosis in office, coordinates and monitors patients progress in fulfilling presurgical duties, monitors hospital stay and follow patients through 90 days post discharge. Guides patient and family through the health system from diagnosis, testing, treatment and followup care.
  • Serves as an essential link between patients and all other healthcare providers.
  • Facilitates appointments for appropriate consultations and support service within established protocols.
  • Develops patient education programs and educates the patient and/or family regarding the patient's clinical condition, treatment, postoperative course, warning signs of potential complication, and the patient's role in recovery.
  • Advocates for the unique needs of each patient to ensure that needs are met using facility and community resources.
  • Provides emotional support and counseling regarding the orthopedic surgery.
  • Encourages patient independence and selfcare.
  • Gathers clinical information from EMR, and performs telephonic health assessments to determine patients risk score.
  • Documentation of clinical assessments and care plans.
  • Educates patients and caregiver on surgery processes and sets clear expectations for discharge and selfcare.
  • Develop and implement care plans, facilitate care transitions, and communicate updates to care team and patient.
  • Collaborate with multidisciplinary team members in acute, postacute and outpatient settings to identify patient needs such as home health, DME, physical therapy and community services.
  • Follows up on patients at predetermined intervals and as needed based on patient status either telephonically or onsite.
  • Completes and submits all required documentation within established guidelines.
  • Tracks and evaluates care plans and revises on ongoing basis as determined by patient's health status and communication of those changes to all other disciplines involved in the patient's care.
  • Monitors LOS and ancillary resource used on an ongoing basis. Takes action to achieve continuous improvement in both areas.
  • Identifies care gaps or barriers to ensure that discharged patients receive appropriate resources and services.
  • Remains flexible and responsive when changes occur in patient activity and workload. Adherence to the scope of practice for the Registered Professional Nurse per state regulatory guidelines.
  • Perform other duties as assigned.
