Overview:
We are looking to hire a
Program Coordinator for our HIV Services.

The Program Coordinator works closely with the HIV Program Manager to coordinate and expand LifeLong's HIV Care and Prevention services at LifeLong health centers in Alameda and Contra Costa County.

This position includes both direct contact with patients as well as operational planning and management functions. Direct services will be at a reduced level to accommodate the administrative functions of this role.

This is a full time, grant funded, benefit eligible position.

This position is represented by SEIU-UHW.

Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Responsibilities:


  • Intake & Outreach
  • Accept and process referrals into the program, including completion of initial assessments.
  • Use health records and analytics tools to identify patients eligible for the program.
  • Coordinate and conduct outreach to promote program visibility and support access to care.
  • Establish working relationships and provide ongoing support for collaborations with CBOs and other care partners that provide resources to patients
  • Operational Planning & Management
  • Participate in planning for program operations, expansion and sustainability
  • Identify training needs and coordinate training programs to expand LifeLong's capacity to provide quality care to the population served; Assist with onboarding and training new program staff
  • Orient staff to Epic workflows to ensure consistent, reliable documentation
  • Promote diversity, equity and inclusion in support of patients and staff
  • Engage the voices of patients and/or caregivers in the design and delivery of services
  • Program Monitoring, Evaluation and Quality Improvement
  • Provide ongoing program oversight to ensure consistency and quality of services; may include regular internal audits to ensure program documentation, charts, and reports are accurate and complete
  • Support quality improvement efforts including tracking of performance indicators; test and implement changes to meet established goals, including patient satisfaction and clinical outcome measures
  • Review and maintain uptodate and accurate knowledge of best practices in care for the population served, which may include emerging research
  • Support and troubleshoot billing and/or documentation issues to optimize program revenue
  • Support with grant reporting and invoicing
  • Case Management
  • Assist team with patient panels, aiding with linkage to relevant services and education; and communicate with patients' care team members, external partners, and identified social supports.
  • Meet and engage with patients to build effective relationships and assess strengths and needs through use of standard screening tools and health and social services records review
  • Understand the program's functions to support and cover for other program staff as needed.
  • Other duties as assigned by Program Manager

Qualifications:

  • Commitment to the provision of integrated HIV prevention and treatment services for lowincome and unserved populations.
  • Demonstrated ability and sensitivity to working with diverse lowincome populations with various educational, lifestyle, ethnic, and cultural origins.
  • Strong organizational, administrative, multitasking, prioritization, and problemsolving skills.
  • Ability to be flexible and adaptive to change as well as persistent in completing complex tasks.
  • Demonstrated effectiveness in managing projects, meeting deadlines, facilitating teams, and working collaboratively with various stakeholders.
  • Clear and concise communication skills written and oral as well as exceptional interpersonal skills.
  • Ability to effectively present information to others, including other employees, community partners and vendors.
  • Ability to work with individuals and organizations at the local level to build support.
