- Preferred: 2 years' experience in care coordination, Medicaid billing, or case management. Preference given to candidate that has or can obtain a ND Social Work License,
- Possess a valid driver's license
- Successful completion of ND Care Coordination Training within 6 months of employment
- Successful completion of background check
Job Summary:
Care Coordination empowers individuals to reach and maintain their highest level of physical and mental well-being. As a 1915i Manager, you will regularly meet with individuals involved in the 1915i Care Coordination Medicaid Amendment Plan. The 1915i Manager will assess the client's needs and develop a participant-centered care plan and client-driven goals. The 1915i Manager will work with local agencies and care teams to ensure success in the realms of recovery, housing, employment, and recidivism. The 1915i Manager is expected to meet with participants once a month and document each interaction. The 1915i Manager will be responsible for the supervision of the 10-month care coordinators and Medicaid Billing Specialist.
Responsibilities:- Assist in ensuring care coordinators and other service providers have the necessary training and supervision in service delivery.
- Keep abreast of changes in legislation, policy, and service modalities while relaying that information to the team.
- Work closely with the Medicaid Billing Specialist to ensure that services are being delivered efficiently and accurately, that service caps are not being reached and that documentation is completed in a timely manner.
- Onboard employees and participants to ensure that required documentation, training, and assessments are completed.
- Complete comprehensive assessment and reassessment activities.
- Develop an individualized person-centered plan of care including the crisis plan component based on the information collected through the assessment.
- Crisis plan development, implementation, and monitoring.
- Referrals, collateral contacts, and related activities.
- Monitoring and follow-up activities.
- HCBS settings rule compliance verification.
- Eligibility redeterminations.
Work Environment and Physical Demands: While performing the duties of this job, the employee is frequently required to sit, walk, and stand. The ability to hear and see in order to communicate effectively as well as communicate clearly and concisely, both orally and in writing; Maintain effective audio/visual discrimination and perception to the degree necessary for the successful performance of assigned duties; Maintain mental capacity, allowing for effective interaction and communication with others. Ability to manage emotions and provide emotional support during stressful circumstances.
Term of Employment: 12 Month Classified Exempt
Evaluation: Annual written evaluation by the Assistant Superintendent of Elementary Education.
