Minimum 1+ years pharmacy experience required.We are seeking a qualified Clinical Pharmacist for a 13-week contract position in Santa Cruz, CA The role involves providing pharmaceutical care to patients in an esteemed Medical Center, focusing on drug monitoring with an emphasis on appropriateness, safety, and cost-effective therapy. The Clinical Pharmacist actively participates in quality improvement activities related to medication use throughout the medical center and may serve as a staff pharmacist up to 50% of the time as assigned. Location: Santa Cruz, CA 95065
Job Responsibilities:
1. Pharmaceutical Care: Provide comprehensive pharmaceutical care to patients at the Medical Center, ensuring the appropriateness, safety, and cost-effectiveness of drug therapy.
2. Clinical Functions: Perform clinical duties related to drug monitoring, with a focus on patient outcomes, in accordance with established protocols and standards.
3. Quality Improvement: Actively participate in quality improvement activities related to medication use throughout the medical center, contributing to the enhancement of overall patient care.
4. Staff Pharmacist Duties: Fulfill responsibilities as a staff pharmacist up to 50% of the time as assigned, which may include dispensing medications, verifying prescriptions, and counseling patients.
6. Documentation and Reporting: Maintain accurate and thorough documentation of pharmaceutical care activities, adhering to regulatory and organizational standards. Prepare reports as required.
7. Collaboration and Communication: Collaborate with healthcare professionals, including physicians and nurses, to ensure seamless integration of pharmaceutical care into patient treatment plans. Communicate effectively with colleagues, patients, and other stakeholders to promote the safe and effective use of medications.
Qualifications:
- Possess a PharmD degree from a US accredited School of Pharmacy
- Have a minimum of two years of experience in a hospital pharmacy or completion of an ASHP-accredited residency program
- Board certification in a specialty by ACCP or equivalent qualification is mandatory
- Current registration in California as a pharmacist (RG PHARMST; PHARMACIST DEGREE - PHRMDG) is a prerequisite
