GENERAL SUMMARY:

There is room for growth in the role of a Full-Time Registered Nurse with PROHealth. This role is based on performance within the company and commitment to the company and its values.


PRINCIPAL DUTIES AND RESPONSIBILITIES:


  • Provides services in accordance with the plan of care
  • Meets Daily Productivity Standards
  • Initiates the plan of care and necessary revisions
  • Provides those services requiring substantial specialized nursing skills
  • Initiates appropriate preventative and rehabilitative nursing procedures
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
  • Participates in Patient Case Conferences
  • Informs personnel of changes in the condition and needs of the patient
  • Counsels the patient and family in meeting nursing and related needs
  • Participates in and presents inservice programs
  • Understands and adheres to established Agency policies and procedures
  • Processes orders and notifies physicians of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
  • Determines the amount and type of nursing needed by each individual patient.
  • Gives total patient care as needed.
  • Completes and submits OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in condition in accordance within 72 hours through our EMR system: _KanTime _(Training is provided, as needed)
  • Appropriately utilizes ICD codes
  • Ensures that documentation is submitted timely and accurately.
  • Performs other duties as assigned.

EDUCATION, EXPERIENCE and LICENSES:


  • Graduate of an accredited school of professional nursing and currently licensed in the State of California as a Registered Nurse (RN).
  • One (1) year of nursing experience in a Home Health setting.
  • Must have current BLS Certification.
  • Driver's License, Active Auto Insurance, and Reliable Transportation is required.

SKILLS AND ABILITIES:


  • Advanced knowledge of medical terminology
  • Excellent communication skills; ability to be flexible and work well with others.
  • Ability to prioritize and multitask.
  • Ability to maintain confidential information.
