Intake Specialist - Floral Park, United States - CareLink Home Care

Description

The Intake Specialist will be responsible for handling the intake process for new clients, including gathering necessary information, assessing eligibility, and coordinating care plans.

They will work closely with the Care Management team to ensure a smooth transition for clients and provide excellent customer service.

• Receive and process referrals from healthcare providers, families, and clients
• Conduct initial screenings and assessments to determine client eligibility and care requirements
• Work with multiple departments to schedule and organize client appointments.
• Verifying medical insurance coverage and benefits for each patient
• Maintain accurate and uptodate client records and documentation
• Provide ongoing support and communication to clients and caregivers
• Perform clerical work such as filing, copying and faxing
• Collaborate with internal teams to streamline the intake process and improve efficiency
• Other duties as assigned by Director.

• High School Diploma
• Previous experience in a similar role at a highvolume homecare agency or healthcare setting
• Knowledge of homecare services, care coordination, and regulatory requirements
• Strong organizational skills and attention to detail
• Excellent communication and interpersonal skills
• Ability to work effectively in a fastpaced environment and prioritize tasks
