- Participate in the development of the individualized plan of care.
- Provide exceptional Physical Therapy - PT to patients by assessing and understanding care plan.
- Evaluate patient's functional status and home environment for hazards or barriers to more independent living. Identify equipment needs.
- Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
- Submits all clinical documentation, time records and other information in the required time frame.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Supports company mission by providing the highest quality of home care.
- Licensed Physical Therapist-PT in the state of New York with at least 1 year of clinical experience; Home Care preferred
- COVID Vaccinated
- Proven work experience as a physical therapist
- Graduate degree or greater in physical therapy
- Valid license to practice physical therapy
- Oasis experience strongly desired
- Have valid driver's license, reliable transportation and willing to travel
- Willing to take advantage of our excellent training system comprised of in house and field orientation
- Have great communication skills, excellent organizational, and time management skills

- Ability to work independently with minimal supervision.
