Currently, we are seeking BE/BC Internal Medicine physicians to join our Atrium Health Union West Transition Services and Internal Medicine team in Matthews, NC.
Joining Atrium Health Union West Transition Services and Internal Medicine means becoming part of an outpatient transition services clinic with a clear mission: helping patients restore their health and well-being. Our Transition Clinic specializes in ambulatory Internal Medicine for patients facing high risk due to medical complexity or limited healthcare access. We offer comprehensive care with visits from providers, pharmacists, and social workers. During our 30-day follow-up period, we identify and address health barriers by focusing on patient engagement, access, and education. Our care is delivered in-office, virtually, or with community paramedicine support, depending on the patient's needs. This is complemented by a traditional Internal Medicine clinic, ensuring patients receive top-notch clinical care, extensive support, and a patient-centric experience in an environment designed for wellness.
