- Deliver home-based care to patients dealing with illnesses, injuries, and chronic conditions, typically post-discharge from a hospital or rehabilitation center
- Collaborate with a team of physicians and supervise LPNs, CNAs, and aides involved in patient care
- Conduct patient assessments, document progress, and communicate any changes to the attending physician
- Educate patients about their care plan, prescribed medications, therapies, dietary requirements, and exercise routines
- Provide guidance to family members on the patient's care plan, medications, and household safety measures
- Ensure treatment goals are achieved and facilitate the patient's discharge from services
- Active Registered Nurse (RN) License in the state of Colorado
- Graduation from an accredited nursing school or program
- Minimum of 1200 hours of experience as an RN
- Certification in Health Care CPR/BLS (completion of a demonstration course is required)
- Preferably, prior experience in home health care
- Demonstrated strong organizational capabilities
- Proficiency in managing patients with chronic ailments
- Successful completion of a background check and drug test (THC screening is mandatory)
- COVID vaccination not mandated
