    Registered Nurse (RN) Case Manager

    Opportunities in Fort Collins, Colorado

    Compensation: $30-$150 per visit (varies based on visit type), PLUS additional mileage and travel compensation with on-call requirement

    As a Home Health Registered Nurse, here's an overview of your responsibilities:

    • Deliver home-based care to patients dealing with illnesses, injuries, and chronic conditions, typically post-discharge from a hospital or rehabilitation center
    • Collaborate with a team of physicians and supervise LPNs, CNAs, and aides involved in patient care
    • Conduct patient assessments, document progress, and communicate any changes to the attending physician
    • Educate patients about their care plan, prescribed medications, therapies, dietary requirements, and exercise routines
    • Provide guidance to family members on the patient's care plan, medications, and household safety measures
    • Ensure treatment goals are achieved and facilitate the patient's discharge from services

    Essential qualifications for Home Health Registered Nurses:

    • Active Registered Nurse (RN) License in the state of Colorado
    • Graduation from an accredited nursing school or program
    • Minimum of 1200 hours of experience as an RN
    • Certification in Health Care CPR/BLS (completion of a demonstration course is required)
    • Preferably, prior experience in home health care
    • Demonstrated strong organizational capabilities
    • Proficiency in managing patients with chronic ailments
    • Successful completion of a background check and drug test (THC screening is mandatory)
    • COVID vaccination not mandated
