Job Summary:


We are seeking a dedicated and compassionate Admissions Nurse to join our growing team at United Hospice & Palliative Care of Arizona.

In this pivotal role, you will be the first point of contact for patients and families as they transition into our hospice services.

Reporting directly to the Clinical Director, you will be responsible for performing comprehensive assessments, initiating care plans, and serving as a liaison between patients, families, and our interdisciplinary team.


Key Responsibilities:


  • Perform comprehensive patient assessments to establish eligibility for hospice services
  • Develop initial nursing care plans and coordinate with the interdisciplinary team
  • Educate patients and families on the services and benefits of hospice care
  • Act as a liaison between referral sources, patients, and the hospice care team
  • Collaborate with Clinical Director to ensure a smooth admissions process
  • Maintain accurate and timely documentation in our electronic medical records system
  • Comply with federal, state, and organizational policies, procedures, and regulations

Pay:
$88, $92,000.00 per year
