- Identify and enroll patients eligible for RAP program.
- Conduct intake, comprehensive assessments/re-assessments and evaluations for Rap patients.
- Develop and monitors care plans for RAP patients.
- Develop linkages and refers patients for additional services.
- Provide appropriate follow up on patient referrals, missed appointments etc.
- Facilitate patients who are no longer eligible for RAP or to move between RAP Tiers.
- Provide health education and risk reduction to patients within scope of discipline to assist in patient engagement and retention and reduce barriers to care to enhance health outcomes.
- Develop and implement retention in care strategies.
- Provide ongoing follow up and support to patients who are not virally suppressed.
- Participate in and/or facilitate case conferences.
- Maintain data and case records as required and prepares necessary reports.
- Participate in ongoing training and regional collaboratives to enhance knowledge and skills.
- Participate in Quality improvement trainings and projects.
- Work collaboratively with entire Genesis team.
- Implement evidence-based programs to improve health outcomes of patients.
- Vocational/Technical School required, Bachelor's degree preferred
- Valid New York State LPN License, New York State RN License preferred
- Minimum 2-4 years experience
- CPR Certification
- Infection Control Certification

Sun River Health is currently looking to fill a full time Adherence Nurse (LPN or RN) position at our busy Poughkeepsie, NY site.
The Adherence Nurse will provide nursing care, medical case management, adherence education, support, monitoring and follow up services to HIV positive patients.
This position will render nursing care to patients as directed by the medical provider pursuant to the protocols of HRHCare and Works cooperatively and collaboratively with the entire Genesis and medical teams in various sites.
