Our patients have access to support services that include nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations.

_ Responsibilities and Functions:
_


  • Conduct intake and benefits screenings, assessments, complete service plans, and required progress note documentation on all assigned patients promptly hours is standard).
  • Screen all CCC clients for benefits at a minimum of every six months to determine benefit needs and enroll them in benefits specialty services as appropriate.
  • Provide resources and benefit referrals for patients as needs arise.
  • Track all services provided on a benefits log that is to be submitted weekly to the direct supervisor and enter all data and services as required into Case Watch by the 15th of each month. Services on logs need to match Case Watch data entry and notes. Complete fee tickets with weekly logs.
  • Attend community meetings and related trainings as required.
  • Attend meetings or supervision to discuss patient cases and contractual requirements regularly. Participate in regular chart reviews and correct all findings as indicated.
  • Maintain individual record documentation as required by DHSP contract standards.
  • Maintain appropriate caseload as determined by direct supervisor and DHSP contract standards.
  • Maintain monthly productivity rate per DHSP standards.
  • Advocate and assist patients in obtaining all needed medical insurance benefits they are eligible for, including but not limited to, Medi-Cal, Medicare, medical insurance through Covered California, VA benefits, and private insurance. And also provide referrals for medication coverage for the uninsured and underinsured.
  • Provide benefits management to patients currently enrolled in benefits services to ensure that they continue to receive and keep their benefits and follow up and document on these current clients regularly.
  • Assist patients in transitioning, from Medi-Cal Fee for Service to new Medi-Cal Managed Care and in choosing appropriate HMO plans.
  • Provide workshops and outreach education on benefit services, as required by DHSP contract and targeted population needs.
  • Assist patients with appeals processes due to denial of benefits. As required, work with any legal representation, retained by patients for this purpose.
  • Participate in regular Quality Improvement and case conference team meetings with the HIV Care team.
  • Participate in regular chart reviews and follow up with all recommended corrections.
  • Travel from site to site depending on client needs.
  • Perform other related duties as assigned.

_ Requirements and Qualifications:
_


  • Minimum of one year experience making eligibility determinations and assisting clients in accessing public benefits or public assistance programs or similar experience.
  • Basic knowledge of HIV/AIDSrelated issues.
  • Bachelor's Degree in related field is preferred.
  • Bilingual in English and Spanish preferred.
  • Ability to work as a team
  • Computer/data entry basic skills.
  • Must relate well to all cultural and ethnic groups in the community.
  • Detailoriented with strong organizational skills.
