The Family Resource Specialist I will provide prenatal and parenting education and support for parents and their children (birth - 5 years) through one on one visits in the home,Responsibilities
- Provide one on one visits in the home, frequency based on assigned level, using the curriculum to educate and support parents regarding prenatal education (healthy pregnancy, child birth), breastfeeding, parent/child interactions, child development,safety sexual health/family planning and parent/child health.
- Engage in tasks necessary to the provision of direct service including but not limited to paperwork digital record keeping documentation assessments monthly service plans progress reports transition plans discharge summaries completed according to agency policy/procedure driving to from home appointments occasional client transport utilizing technology conduct home via Zoom Facetime or phone as necessary appropriate attending community meetings families agency researching knowing community resources refer families responding participant emergencies time frames NCHS HFA Policies Procedures collaborate agencies service providers informal supports advocate families case managers therapists relatives responding family referrals conduct surveys time frames NCHS HFA Policies Procedures initiating contact assigned specialist appropriate time outlined NCHS HFA Policies Procedures participate family engagement efforts get keep actively involved program participate supervision sessions week hours attend participate various team Children Family Center building agency employee meetings collect statistics implement CQI procedures defined program assignment complete training perform position professional growth Foundations Parent Survey Growing Great Kids Prenatal-36 month GGK Preschool Mothers Babies Child Abuse Neglect Cultural Humility facilitate team atmosphere partnership other agency staff promote positive working environment represent agency programs clients community organizations public general show flexibility work schedule determined supervisor evening weekend work required
