The Family Resource Specialist I will provide prenatal and parenting education and support for parents and their children (birth - 5 years) through one on one visits in the home,Responsibilities
- Provide one on one visits in the home frequency based on assigned level using curriculum to educate and support parents regarding prenatal education healthy pregnancy child birth breastfeeding parent/child interactions child development safety sexual health/family planning parent/child health.
- Engage tasks necessary direct service including but not limited to utilising curriculum plan individual lessons each family paperwork digital record keeping documentation assessments monthly service plans progress reports transition plans discharge summaries completed agency policy/procedure driving visits appointments occasional client transport utilizing technology conduct visits via Zoom Facetime phone necessary appropriate attending community meetings behalf families agency researching knowing resources refer families responding participant emergencies person time frames NCHS HFA Policies Procedures collaborating agencies services informal supports case managers therapists relatives advocate behalf families conducting surveys assigned time frames NCHS HFA Policies Procedures initiating contact assigned Family Resource Specialist appropriate time time outlined NCHS HFA Policies Procedures participating family engagement efforts get keep actively involved program supervision sessions week least hours attending participating team Children Family Center building agency employee meetings collecting statistics implementing CQI procedures defined program assignment completing training perform position professional growth Foundations Parent Survey Growing Great Kids Prenatal-36 month GGK Preschool Mothers Babies Child Abuse Neglect Cultural Humility facilitating team atmosphere working partnership staff promoting positive working environment representing agency programs manner clients community organisations public general showing flexibility schedule supervisor evening weekend work required
