Description

(Healthcare experience is a plus but not required) Position offers a base salary with a commission & bonus incentive linked to performance.

Responsibilities:

Research and network with facilities, senior living communities, senior centers, hospitals, senior service providers, senior care professionals, etc. as directed by Owner/Executive Director.
Attend networking meetings, socials, seminars and conferences
Schedule care conferences and any relevant meetings pertaining to discharge planning
Document networking activities in software
Follow up with all contacts on a regular basis
First point of contact for inquiries and prospects for admission when at a facility/center/hospital community etc.
Frequently speaking with business office and other staff to ensure accurate care plans
Coordinates client care transition with hospital case manager, team members
Keeps other members of the care team informed of challenges which might delay the patients discharge and works collaboratively to resolve challenges
Instructs patient and family on the plan of care, and refers to community resources as necessary

Qualifications:

2+ years proven sales and marketing experience
Full Time Monday-Friday
Organized
Excellent communication skills
Bachelor's degree
Ability to work and make decisions independently
Appearance, behavior and performance must be professional at all times.

Compensation:
$60K-$70K base salary + Tier commission structure + Bonus
