    Description

    Registered Nurse - Sign On Bonus

    Department: Utilization Management - UH

    Job type: Part Time

    Shift: Weekend Days

    Qualifications:

    • Graduate of nationally accredited nursing program
    • Current RN license in NM or as allowed by reciprocal agreement by NM
    • CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
    • 1 year directly related experience
    • Bilingual in English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (preferred)

    Responsibilities:

    • Coordinate all systems/services required for an organized, multidisciplinary, patient centered care team approach
    • Assure quality, cost-effective care for the identified patient population
    • Manage the course of treatment of patients and coordinate care with physicians, nurses, and other staff to ensure quality patient outcomes are achieved
    • Conduct initial and ongoing assessments, initiate disease management protocols, and determine and manage outcomes
    • Ensure continuity of care through discharge planning, utilization of resources, and analysis of variances
    • Function as a contact person for patient, family, health care team members, community resources, and employees as necessary
    • Monitor and evaluate short-term and long-term patient responses to interventions and maintain interdependent follow-up as necessary
    • Participate in orientation, continuing education of staff RN's and other health care team members as appropriate
    • Participate in continuous quality improvement activities by evaluating patient care systems that may include standards, protocols, and documentation
    • Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
