Our focus is on delivering skilled professional healthcare services such as nursing, physical therapy, occupational therapy, speech therapy, medical social work, and non-skilled caregiver services in a home setting.

    We work with many home health agencies, currently supporting a range of counties including San Francisco, Alameda, Contra Costa, Santa Clara, San Joaquin, San Mateo, Stanislaus, San Benito, Madera, Solano, Napa, Sonoma, and Marin.

    Key Responsibilities:

    • Develop individualized therapy treatment plans for home health patients.
    • Implement standardized assessments to accurately diagnose various physical, cognitive, and communication disorders.
    • Establish actionable treatment goals and milestones together with patients.
    • Maintain timely documentation for treatment plans and progress reports.
    • Supervise PTAs and COTAs as needed.
    • Communicate patient progress effectively to relevant stakeholders.
    • Conduct research on related physical and functional challenges.

    Qualifications:

    • Active California Professional License is a must.
    • Minimum of 2 years of clinical experience as a therapist, including experience in diverse settings.
    • Preferably, have prior experience in a home health environment; however, we offer training for the right candidate.
    • Current CPR certification is required.
    • Possess a valid driver's license with an insured automobile in good working order.

    Required Skills:

    • Meet physical demands associated with the role.
    • Exhibit the ability to build rapport with patients easily.
    • Possess excellent written and verbal communication skills.
    • Maintain a positive and patient demeanor even in stressful situations.
    • Demonstrate strong organizational and leadership abilities.
    • Basic computer and typing skills are necessary.
