Job summary
The Occupational Therapist plays a vital role in supporting older adults enrolled in the PACE model.Working under the supervision of the PACE Center Manager, the OT practices independently while collaborating closely with the interdisciplinary team (IDT) to deliver holistic, participant-centered care.
This role blends clinic-based therapy, home safety assessments, care planning,
and functional rehabilitation to help participants remain safe,
independent,
and engaged in their communities.
