We are currently seeking a compassionate and experienced Nurse Practitioner to join our interdisciplinary team and contribute to our mission of delivering exceptional hospice care.


Responsibilities:


  • Conduct comprehensive assessments of patients to determine their physical, emotional, and psychosocial needs, with a focus on pain and symptom management.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team to address the holistic needs of patients and families receiving hospice care.
  • Provide expert guidance and education to patients, families, and caregivers on pain management, symptom control, and endoflife care.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and effective patient care.
  • Monitor and evaluate patient responses to treatment, adjusting care plans as necessary to provide optimal symptom relief and comfort.
  • Document all patient interactions, assessments, and interventions accurately and in a timely manner, in accordance with organizational standards and regulatory requirements.
  • Participate in ongoing education and training to stay updated on best practices in hospice and palliative care.

Qualifications:


  • Master's degree in Nursing from an accredited institution.
  • Current Nurse Practitioner certification and state licensure.
  • Minimum of two years of experience in hospice, palliative care, or a related field.
  • Solid understanding of pain management and symptom control in patients with lifelimiting illnesses.
  • Excellent interpersonal, communication, and collaboration skills.
  • Compassion, empathy, and a commitment to providing highquality, patientcentered care.
  • Ability to work effectively in a fastpaced and emotionally demanding environment.
