- Conducts client intakes.
- Assesses clients and formulates individual care plans that comply with the Department of Elder Affairs (DOEA) and Agency for Health Care Administration (AHCA) requirements.
- Maintains client confidentiality and handles client files in compliance with HIPPA requirements.
- Maintains client charts to include all necessary documentation.
- Maintains and submits program statistical reports as required.
- Re-evaluates the needs of each client and document in their care plan on a quarterly basis.
- Monitors client vital signs and reports any changes to responsible party or physician as necessary.
- Counsel's participants and caregivers regarding topics related to health and well-being.
- Develops and encourages program activities that promote client mental and physical stimulation, self-esteem, and social skills.
- Creates a safe, comfortable, and welcoming atmosphere.
- Supervises staff and volunteers to assure appropriate interaction and activities with clients.
- Provides and/or participates in necessary training.
- Attends staff meetings, conferences, workshops, and seminars as required.
- Maintains State required CEU's for RN licensure and CPR certification.
- Performs related work as required.
- During emergencies, all employees are considered essential personnel and may be required to perform alternate duties.
- Associate or Bachelor's degree in Nursing from an accredited College or University.
- Possess a current State of Florida RN license in good standing.
- Possess a current CPR/AED certification.
- Must complete 24 continuing education units every two years to include State of Florida course requirements to maintain RN licensure.
- Minimum 4-years nursing experience, 2-years in a supervisory position working with Alzheimer's and dementia patients.
- Excellent interpersonal and communications skills.
- Strong command of English both oral and written.
- Must have the ability to work well with others.
- Must have the ability to manage multiple priorities and possess good organizational skills.
- Must possess computer skills.
- Lifting, stooping, bending, pushing and assisting clients to stand.
- Full use of upper and lower extremities and back muscles.
- Use of proper body mechanics to avoid back injury.
- Good hearing and eyesight with or without corrective lenses.
