Job Summary


In this position, you will be a vital part of our care team, providing direct patient care under the supervision of a registered nurse.

Your responsibilities will include following the plan of care, administering treatments, and collaborating with team members to ensure positive patient care outcomes.

You will have the opportunity to empower patients to improve their self-care while documenting all information through a provided electronic device (smartphone).


How You'll Spend Your Time in This Role:

  • Providing direct patient care as defined in the State Nurse Practice Act
  • Implementing nursing practices based on a comprehensive assessment and the plan of care
  • Documenting patient services accurately and in a timely manner
  • Educating patients, families, and caregivers about the diagnosis and selfcare plan
  • Coordinating hospice services with the interdisciplinary team
  • Using equipment and supplies effectively and efficiently
  • Ensuring appropriate pain and symptom management
  • Participating in personal and professional growth and development activities
  • Engaging in quality assessment and performance improvement initiatives
  • Performing other duties as assigned by the registered nurse

What We're Looking For:


Qualifications Include:

  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, with preference given to community health/hospice or medical/surgical experience
  • Current LPN/LVN license in the State
  • Strong verbal and written communication skills
  • Excellent organization skills
  • Valid driver's license and a reliable, insured automobile in good working order
  • Current CPR certification
  • Understanding of the philosophy of hospice and the needs of the terminally ill
  • Participation in Case Conferences
