Program Assistant/Driver Per Diem (113)
Los Banos, CA

Supervise train/coach consumers with intellectual and physical disabilities on social skills as outlined by their individual integration plan.

Provide training/coach consumers with intellectual and physical disabilities in the areas of daily living skills (e.g., social, domestic, community integration, vocational and self-care) Coach/train consumers in an independent living environment with preparing meals, self-care needs, grocery shopping, house hold needs, and medications.


  • How will I contribute_
  • Assisting consumers with total care of bowel and bladder.
  • Providing physical assistance and supervision according to the dietary needs of the consumers.
  • Teach, coordinate, and support consumers with community integration while monitoring their health and safety.
  • Maintain knowledge of the principles of each consumer service needs and or behaviors to assist them with accomplishing individual goals.
  • Daily charting and data collection on progression and regression of consumer objective goals.
  • Report any problems to your supervisor timely to allow for any concerns to be addressed timely.
  • Develop strong working relationships with community agencies, program supervisor, care providers, peers, and family members.
  • Represent and positively promote program to funding source, community agencies, families, and related groups.
  • Provide coaching and redirection to consumers with behavior challenges during crisis situations.
  • Complete and report all consumer related incidents, accidents, behaviors.
  • Follow and operate according to all HIPAA regulations.
  • Capable of working in a fastpaced environment.
  • Maintain complete confidentiality of consumer information always.
  • Provide summaries of clients' progress in the community and report any problems to your supervisor timely to allow for any concerns to be addressed timely.
  • Attend monthly and quarterly mandatory staff meetings.
  • Complete required paperwork in the event of incidents/accidents to your supervisor.
  • Follow the company policies and procedures while maintaining and following Title 22 regulations.
  • Maintain professionalism in the workplace at all times.
  • Submit timecard and expense sheets to the supervisor according to the timelines stipulated in policy.
  • Create and follow a lesson plan according to required curriculum outline.
  • Keep a clean working area, occasional cleanup of facility including changing rooms and bathrooms.
  • Maintain consumer's outlined medical plan as directed by physician.
  • Maintain a safe environment for the consumers; prevent harm to consumers, self, and others.
  • Work and create a teamwork environment, supporting ideas, creating ideas, and execution of projects.
  • Complete and maintain current CPR/FA certification.
  • Report any safety concerns to management in a timely manner.
  • Follow outlined program plans in relation to behavioral programs.
  • Other duties as assigned by supervisors.
  • What I bring_
  • High School Diploma (or Equivalent)
  • CPR certified
  • First Aid certified
  • CNA desirable
  • Class C Driving License
  • Knowledge in utilizing assistive devices (e.g. mechanical lifts, wheelchairs, walkers, hearing aids, adaptive feeding utensils) Knowledge of software, Microsoft and Excel.
  • Be flexible to cross train all departments to meet the needs of all consumers. Problem solve during crisis situations
