CWC is currently seeking a
full time, part time, contractual Social Workers and Professional Counselors to join our Hollistic outpatient mental health center.

Compassionate Wellness Center, LLC is a CARF accredited OMHC serving Washington County and surrounding areas.

Our Comprehensive Treatment Center is unique in that we provide a variety of services for individuals working on mental health matters through individual and group therapy; and goal setting and learning life skills through our psychiatric rehabilitation.

These services are for clients ages 5 and up.

Our substance abuse services range from addiction counseling, individual and group therapy, We now provide IOP services as well.

Moreover, to complement the above blend of therapies and services, we promote the use of safe and effective medication management, as needed, Palliative Medicine providing a multidisciplinary approach to patients with pain and other symptoms and Advanced Care Planning.

Who are we looking for:
_
LCSW-C, LMSW, LCPC, LMSW, LGPC****_
Responsibilities _**- Conducts individual counseling sessions and document as required; group and/or family sessions, and documents as required;

• Facilitate assigned group or family sessions ; Perform initial assessment and follow up assessment;
• Prepares individual treatment plans for each assigned patient;
• Ensures all documentation regarding patient care, treatment and incidents is completed timely and in a clear and concise manner;
• May act as a liaison between referral sources and patients;
• Provides crisis intervention to patients, as needed;
• Complete discharge summaries upon discharge;
• Evaluates patient's needs and determines if referrals to other programs or facilities as needed

Qualifications

• As an
  _LMSW_ you must have completed a Masters Level Social Work program and successfully passed the licensing exam;

As an _LCSW -C_**_ _You must you must have completed a Masters Level Social Work program and successfully passed the licensing exam


As an LCPC - you must have Licensed Clinical Professional Counselor license
