At Care Plus Hospice, we are seeking a motivated and compassionate LPN to work as an Intake Coordinator with supplemental On-call and Per diem visits to service our patients in the Scottsdale, AZ areas and Metro Phoenix.

• Participates in coordination of hospice services and assigning staff members based on identified needs.
• Provides accurate and timely documentation of all consents to the Admission department.
• Support quality assurance and quality improvement
• Provides other administrative assistance and support duties as directed by the Director of Patient Care Services.
• Provides appropriate followup to all patients, families, and facility staff
• Complies with hospice policies and procedures, laws and regulations, standards of practice, and established work rules.
• Maintains accurate tracking systems to review documentation and ensure compliance.
• Communicates with staff and outside agencies as directed.
• Facilitates communication between patients/families and other team members.
