- Minimum two (2) year in psychiatric nursing or three (3) year experience as a Charge Nurse required.

- Must frequently utilize physical ability for standing and see with correction for color discrimination and depth perception.
- Must regularly utilize physical ability for sitting, speaking, hearing, and seeing with correction for close, distance and peripheral vision, and focusing ability.
- Mental demands include but are not limited to emotional stability, physical stamina and agility to handle stress and respond quickly and effectively to emergency situations.
- Completes an assessment for each patient by performing a thorough mental, physical, nutritional and functional assessment on admission, and as needed, based upon patient's condition.
- Assesses patient's psychosocial status, and patient/family learning needs and discharge criteria.
- Perform daily clinical reviews with all payer types (Managed Medicare, Managed Medicaid and commercial) to secure authorization for continued treatment (i.e. by fax, telephone or on-line) based on payer's criteria.
- Completes quality and timely appeal/denial letters. Participates in post claim recovery review and ongoing audit activity, supporting compliance with CMS and other regulators.
- Assesses for pain, utilizes pain scale, documents accordingly.
- Organizes and directs care of patients, setting priorities and assigning most skilled staff to care for most acutely ill patients.
- Provides/coordinates education of patients based on assessment of learning needs and readiness.
- Provides care for patients with: Depression/Suicidal impulses, Mania, Delusions/Hallucinations, Paranoia, Anxiety, Aggressive/Assaultive Behavior, Chemical Dependency Issues/Detox, Personality Disorders, i.e. Borderline/Sociopathic, Eating Disorders, Dissociate Identity Disorder/PTSD, Dementia/Alzheimer's, and medical problems such as hypertension, headache, diabetes, URI, UTI, heart disease, thyroid disorders, and acute or chronic infection.
- Intervenes in managing complex patient care situations utilizing departmental policies and procedures that promote most facilitative alternatives. Prioritizes interventions in rapidly changing patient situations.
- Assesses patient's response to interventions; communicates these findings to other team members.
- Sets priorities appropriately utilizing data from Nursing Assessment. Adapts procedures to individual patient needs while maintaining the integrity of the principals involved.
- Documents education about medications, disease symptom identification, social skills, stress management, and impulse control regulation.
- Is available for 1:1 interaction with patients as needed and shares in providing intervention and/or management of inappropriate behavior.
- Anticipates emergent situations and intervenes early and appropriately.
- Identifies patients at risk for injury due to falls or behavior, coordinates the efforts of the team to prevent injury and communicates to others the potential risk of injury.
- Demonstrates effective management of assaultive behavior.
- Demonstrates competence in CPI.
- Utilizes observational skills to provide constructive feedback and input to patients.
- Observes signs and symptoms presented by patients and acts appropriately, effectively and in a timely fashion.
- Reports pertinent information to the appropriate discipline in a manner consistent with the acuity of the situation.
- Completes charting on all assigned patients, in progress notes and all pertinent graphics.
- Plans work to facilitate completion of assigned duties during shift.
- Establishes and maintains a therapeutic alliance with patients by assisting the patient to use the therapeutic milieu.
- Is proficient in special procedures including, but not limited to collecting laboratory specimens, glucometer, and 02 Administration.
- Ensures that lab results and other test results are posted upon receipt.
- Ensures that physician orders are affected within 30 minutes and transcribed within one hour.
- Receives and reports abnormal laboratory values to physicians, documents accordingly.
- Actively attends and encourages participation of patients in milieu activities.
- Respects patient confidentiality in and outside of group setting and uses discretion when discussing patient information.
- Transcribes and administers medications and treatments accurately. Documents correctly and legible on medication record. Practices the 5 Rights of medication administration.
- Ensures that medication consents are obtained and signed prior to administering psychotropic medications.
- Monitors patient for adverse drug reactions and reports any adverse drug reactions immediately to physician or pharmacist. Knows side effects, normal dosage and contraindications of all medications given.
- Demonstrates responsibility for proper safekeeping, monitoring, and accurate recording of all controlled medications kept in outpatient.
- Acts as a role model in demonstrating nursing practice and procedures.
- Demonstrates a thorough knowledge and adherence to Hospital policies and procedures and assists with interpreting Hospital policies.
- Uses good nursing judgment and needs minimal supervision, but able to recognize when needs to confer with doctor, house/nursing supervisor, or program director prior to making decisions.
- Utilizes a manner that is friendly and welcoming.
- Actively promotes strict adherence to all policies and procedures.
- Regularly reviews work in progress, ensuring that all medications and diagnostic tests ordered by the physician are expeditiously carried out and recorded.

ResponsibilitiesRiver Vista Behavioral Health sits along the bluffs overlooking the San Joaquin River, this brand new, 128-bed facility, will provide high-quality and advanced behavioral health services to residents and visitors in the Central Valley. The new hospital will employ more than 250 people, including clinicians, nurses, mental health technicians, support staff and administration.
We are currently seeking a forward thinking and compassionate RN - Outpatient CLINIC (PHP/IOP) to be part of our Outpatient Team. The Outpatient RN is responsible for providing support to the day-to-day treatment and operations of the Partial Hospitalization and Intensive Outpatient programs including, but not limited to: administrative functions, utilization review, assessment, discharge planning, group/individual therapy, case management, and other duties related to delivering a successful program.
QUALIFICATIONS
Education : Graduate of accredited School of Nursing; Current licensure as a Registered nurse thorough the State of California.
Additional Requirements: Thorough knowledge of The Joint Commission, OSHA, and state and federal regulatory requirements. CPR certification and successful completion of Crisis Prevention Intervention (CPI) training. CPI Training may be obtained during new hire orientation.
Knowledge/Skills:
Working conditions for all employees are as follows, but not limited to a psychiatric hospital setting; some risk involved in the event of aggressive patient(s); workload may include day, evening, night, weekend and/or holiday shifts. The Outpatient Registered Nurse is occasionally exposed to outdoor weather conditions, toxic or caustic chemicals, blood borne pathogens, and loud noise levels.
Mental demands described herein are representative of those which must be met by an employee to successfully perform the essential functions of the job but are not limited to the ability to remain calm in a stressful environment; and the emotional stability, physical stamina and agility to handle stress and respond quickly and effectively to emergency situations.
PHYSICAL REQUIREMENTS: Physical requirements include, but are not limited to the ability to communicate effectively with patients, their families, staff and others; the ability to effectively utilize communication equipment; the physical agility to manage patients (ambulatory, non-ambulatory and physically aggressive); the ability to read (i.e. patient charts, written communication, regulations, written policies and procedures, etc.); and the ability to write (i.e. manual charting, written communication, etc.).
