- Prefer high school graduate or equivalent.
- Prefer one-year experience in the provision of care/services to clients in home care setting.
- Must have a criminal background check, other required checks, Nurse Aide Registry & current CPR certification.
- Must be able to read, write in English and follow instructions.
- Current driver's license and own reliable transportation is required.
- Personal care tasks including assist with mobility, bathing/shower, toileting, dressing, eating, continent needs.
- Help with prescribed exercises that the client and In-Home Aide have been taught by a health care practitioner
- Provide or assist with ADL's
- Perform IADL's that are essential to the client's care at home; and record and report changes in the client's condition, family situation or needs to the health care practitioner
- Assist client with self-administration of medications that are ordered by a health care practitioner or other person authorized by state law to prescribe
- Supervision of client's activities.
- Home management tasks.
- Other tasks as assigned within the scope of their role.
- Follows the Plan of Care as written, advises Service supervisor if not able to.
- Documents each visit per Agency policy.
- Communicates ongoing with the Service Supervisor.
- Immediately reports emergency situations by phone to supervisor or office.
- Keeps an accurate time and mileage report.
- Follows assignments and regulations.
- Uses policy manual as guideline for service.
- Works within the organizational channel of authority.
- Wears appropriate clothing and ID badge when at work.
- Participate in the Agency QA Committee.
- Notifying the Agency of absences due to illness, emergency leave, normal vacation periods, or applicable meetings, which will affect agreed service with the Agency.
- Must be able to read 12 point or larger type.
- Must be able to hear and speak in English.
- Must be able to stoop and bend.
- Must be able to travel to prospective clients' residences.
- Must be able to carry bundles weighing up to 10 pounds, upstairs.
