    Job Summary & Deliverables

    Valley EPIC (Evidence-based Programs and Interventions Campus) of Utah is a premier treatment facility designed for men and women in our community who may be struggling with substance use disorders, mental illness, and homelessness. The program offers a full continuum of treatment, from residential and day programs to intensive, traditional outpatient services.

    Valley EPIC involves our newly renovated residential facilities. The campus was designed to integrate substance use treatment approaches with a special focus on trauma-informed care. Our goal is to help people break the chains of addiction, which includes a successful re-entry into the community and a reduction in criminal recidivism.

    The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure and ensures excellence in client care, fiscal management, and regulatory compliance.

    Essential Functions

    • Provides therapeutic behavior management as prescribed by the program-specific care plan in a residential setting
    • Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
    • Assists Therapists or Registered Nurses as needed to develop and review safety/crisis plans and care plans
    • Develops therapeutic relationships with clients
    • Reviews and assesses care plans
    • Coordinates services among internal and external agencies and providers
    • Meets required client care hour expectations and other organizational expectations

    Requirements:
    • Bachelor's degree in Social Work or related field
    • Internship experience as required for SSW license
    • SSW licensure in state of Utah
    • CPR certification
    • Valley de-escalation certification
