- Interpret medication orders and ensure the safe and appropriate dispensing of medications.
- Identify and resolve medicationrelated problems to optimize patient outcomes.
- Provide accurate drug information to healthcare staff, providers, and the community.
- Collaborate with physicians and residents to develop individualized drug therapy plans.
- Establish a pharmacy practice specialty and serve as a resource for education and support.
- Document interventions and outcomes, maintaining thorough records.
- Perform medication order entry in accordance with department policies and procedures.
- Coordinate the daily distribution process within the pharmacy.
- Represent the pharmacy on hospitalwide committees and participate in pharmacy committees.
- Conduct written and inservice education sessions regularly.
- Perform additional projects or duties as required or assigned.
- PharmD degree or Bachelor's degree in Pharmacy from an accredited program.
- NH Pharmacist license or reciprocity pending.
- Board Certified Pharmacotherapy Specialist (BCPS) certification desirable.
- Acute care hospital experience required.
- Knowledge of Cerner preferred.
- Pharmacy residency experience desirable.
