- Who needs the nurse? Father and/or mother
- What care or services are needed? Assistance with personal care or hygiene, care, maintenance and changing of catheters
- Do you have a medical prescription? Yes
- Frequency of service needed Daily
- Indicate the date or approximate dates to start 01/29/2024
- What time do you need the service? Morning (08:00 - 12:00), late afternoon (18:00 - 21:00)
- What days do you need the service? Every day of the week
